What matters

• Preclinical performance still matters, anatomy especially. Plastic surgery is applied anatomy; own it now and every rotation later gets easier.
• Join your plastic surgery (or surgery) interest group, or start one if your school has none. Leadership here compounds into AOA and letters later.
• Find one mentor. A single faculty member or resident who knows your name is worth more than ten cold emails.
• Get your hands moving early: suture workshops, skills labs, practice kits at home.
• Say yes to a research project, even a small one (case report, chart review). The goal this year is momentum, not a first-author PRS paper.

Checklist

• Prioritize anatomy. Aim to be strong, not just passing
• Join (or start) the plastic surgery interest group
• Shadow a plastic surgeon in clinic and in the OR

SThe research summer· Summer between M1 and M2

What matters

• Commit the summer to research: a structured summer program, a mentor’s project, or a database study you can actually finish.
• Choose productivity over prestige: a mentor who publishes with students beats a big name whose projects stall.
• Aim to leave the summer with at least one manuscript moving toward submission and one abstract you could present.
• Start showing your face: local research days, regional meetings, and (if you can) a national meeting like Plastic Surgery The Meeting.

What matters

• Treat Step 1 seriously (you must pass comfortably), but understand Step 2 CK is now the screening number programs see.
• Keep the research pipeline alive. The applicants who stand out have a *line* of work, not one-off projects.
• Meet with your plastics chair or program director (or nearest academic plastics faculty if you have no home program) once or twice this year. Make yourself known early.
• Take a leadership role in your interest group. Leadership feeds AOA selection and your ERAS narrative.
• Keep your hands sharp and shadow ahead of clinical year so M3 doesn’t start cold.

What matters

• Clinical success is a team sport: punctuality, ownership of your patients, and preparation-driven questions beat showing off knowledge.
• Be consistently strong on every rotation, not just surgery. Weak evaluations can undo strong scores.
• Step 2 CK is the number: build a sustainable study system all year rather than cramming before the exam.
• Sub-internship and away-rotation planning starts early. Applications typically open February–March of M3.
• Keep research moving and your CV current; this is the year applications sneak up on people.

What matters

• Good reasons: no home plastics program, a thin research line, a late pivot into plastics, or a specific lab/mentor you want to work with. "Everyone else is doing one" is not a reason.
• Pick the position for mentor productivity and data access (established databases, active clinical projects), not just the institution’s name.
• Set output targets up front: multiple submitted manuscripts, national presentations, and at least one strong letter from the year.
• The hidden value is the network. A research year at a program is a year-long audition, and conference podiums put you in front of the field.

• Decide honestly whether a research year strengthens YOUR application
• Vet the mentor’s recent student output before committing
• Agree on concrete output goals (papers, abstracts) at the start
• Present at national meetings during the year
• Secure a strong letter from the research mentor

M4Aways, ERAS & the Match· Fourth year

What matters

• Choose 2–3 aways strategically: programs you’d genuinely rank highly, with some geographic spread, and look at how many of their residents rotated there as students.
• On sub-Is, own the details: know your patients cold, keep the list tight, be early, help with everything, and treat every day as the interview it is.
• ERAS: letters from plastic surgeons who know you beat bigger names who don’t. Your research line should tell one coherent story.
• Interview prep: be ready to discuss anything on your application, why plastics, and a failure, with specifics and not scripts.
• Have a contingency plan you never need: know how SOAP works, and know what a research year + reapplication would look like.
